Is your Dutch passport or identity card lost or stolen? You need to notify the municipality. You can then apply for a new passport or identity card.

The document has not been confiscated (taken) by the police or a customs official.

You fill in the declaration of loss truthfully. A new passport will be refused if fraud has been committed. Read more on rijksoverheid.nl (in Dutch).

1 new colour passport photo which meets all requirements
If the photo does not meet all the Begin external link: requirements(External link), end external link., the municipality will not be able to submit your application. You will then have to make a new appointment.
A valid form of identification
If you have one. Take all of your identity documents with you. Even if they are expired or from a foreign country.
Your bank card
You will pay at the municipal counter when you apply. You can pay with your bank card. You can also pay in cash only at City Hall Spui. You cannot pay with a credit card.
Apply for a child
Is your child 17 years old or younger? Then your child must also come to the appointment. As of 12 years your child is allowed to come alone. Permission is needed for an identity card for a child up to the age of 12 years and for a passport for a child up to the age of 18 years:
- Everyone with parental responsbility for the child must give permission.
- Will everybody with parental responsibility come to the appointment? Then all of you must take a valid Begin external link: identity document(External link), end external link. to the appointment (not a copy).
- Not everyone with parental responsibility will attend the appointment? At least 1 parent or legal guardian must accompany the child to the appointment. This person takes a completed Begin link: consent form, end link. for the other parent or legal guardian, with a copy of the identity document.
Reporting the lost or stolen document is free of charge. You will have to pay fees to apply for a new document. The fees in 2024 are:

- Report a loss or theft
- The municipality will register the document as missing right away.
- New passport or identity card
- At least 6 working days. The pick-up receipt will state the date as of when you can collect it.
You can pick up your passport or identity card at the city office where you applied for it:
- Pick up your document yourself. Nobody else is allowed to do this for you.
- You do not need an appointment.
- Take your pick-up receipt and your identity document with you (if you have one).

Yes, you can. The municipality has to be able to check your identity. You can use another identity document for this. If you do not have one, an expedited procedure is not always possible. Ask the municipal official what can be done in your situation.

- Indicate this during your appointment.
- Are you applying before 14.00 hrs.? Then your passport or identity card will be ready after 2 working days. For example: if you apply Monday before 14.00 hrs., you can collect it Wednesday.

You can also report the document as missing and apply for a new passport or identity card in the country where you live. You can apply at the Netherlands embassy, the Netherlands consulate or an external service provider such as VFS Global.
No, you cannot use it again. Did you report the document as missing and did you later find it back? It will remain invalid. You will have to turn in the old document. The municipality will destroy it.
