Request attestation de vita extract (proof of life)
The attestation de vita extract is proof of life. A public authority which issues payments can ask you for proof that you are still alive.
- You are registered at an address in The Hague.
- If you are not registered in the Personal Records Database (BRP) or if you need a multi-language extract, first Begin link: make an appointment, end link. at the municipality.

Apply for the extract online (in Dutch). Log in with DigiD. Once you are logged in, click on ‘Volgende’. You can then choose which extract you need.

- You can also apply for the extract at the Department of Public Service counter at Begin link: City Hall Spui or the Leyweg City Office, end link. .
- It is also possible to apply in writing: send your letter to the Begin link: Department of Public Service, end link. .
In person
You can also request an attestation de vita extract at City Hall Spui or the Leyweg City Office. You will get the extract right away if you are registered in the BRP. You need to bring a Begin link: valid form of identification, end link. with you. Make an appointment by phoning tel. 14070.
Multi-language attestation de vita extract
The attestation de vita extract is available in multiple languages. A multi-language extract is in Dutch and French including an attachment with an explanation in German, Spanish, Greek, Italian, Portuguese and Turkish. The extract is not available in English.
Are you requesting a multi-language attestation de vita extract? This is only possible if you are registered in the BRP. You need to visit City Hall Spui or the Leyweg City Office in person with a Begin link: valid form of identification, end link. . To do this make an appointment at the municipality.
€ 16.60 (in 2024). You pay this amount directly through iDEAL when you apply online.
Are you picking up a multi-language attestation de vita at City Hall Spui or the Leyweg City Office? You can pay in cash or use a bank card at City Hall Spui. You can only use a bank card at the Leyweg City Office. Unfortunately it is not possible to pay with a credit card.
- The municipality will send the extract to you within 3 working days. This can take longer depending on the mail service to your home address.
- A multi-language attestation de vita extract will be issued to you on the spot.
- Did you request a free attestation de vita extract? Then the municipality will send the extract within 3 working days after receiving your written request.
- The extract contains your surname, your given name(s), your date and place of birth and your home address.
- Are you about to retire or do you already receive a pension? Then you get this extract free of charge. In this case it is not possible to apply online. Request your free attestation vita in writing to the Begin link: Department of Public Service, end link. . Send a letter which you have signed, a copy of your valid form of identification and a copy of the written request from the pension fund. The extract is only available in Dutch for a written request.

- Do you need to have the document translated? You can have it translated by a sworn translator. For documents which are needed within the European Union you can request a Begin link: multilingual standard form, end link. along with the document from the Netherlands.
